Some evenings and weekends as required (Exempt/Hourly position; 40 hours/week).Job SummaryThe College of Nursing (CON) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services (DMS) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based (HCB) waiver assessment tool for the frail and elderly population and physically disabled. This position will identify, develop and present information educating professional staff, families and individuals seeking HCB Services while also making sound independent determinations as to the level of care and services needed for this vulnerable population.
The majority of in-home assessments will be based in Morgan County, but you may be asked to complete in-home assessments in surrounding counties. You will receive mileage reimbursement and will be given flexibility when scheduling in-home assessments to meet both your needs and client needs. This position requires travel within Morgan County and surrounding counties.
